Dear Teacher or Family Member:
The Life-Span Institute at the University of Kansas supports the practice of protection for human subjects participating in research. We provide the following information for you to decide whether you wish to participate in completing self-determination surveys. If you agree to participate, you will be helping us to create an Adult Version of the Self-Determination Inventory System and may learn more about your student's self-determination. We are asking you to participate, but you can decide not to participate by closing out of the browser or by skipping the survey items on the next pages.
This study contains survey items that will be used to create a new measure of self-determination for youth and young adults between the ages of 13 and 22 years called the Self-Determination Inventory Self-Report measure. It will take about 15 minutes to finish the survey about a student who also answered items about goal setting, problem solving, and taking actions on tasks.
We ask you to participate in our study, but your participation is voluntary. Although we ask for your name initially and who you are filling out the survey about, identifying will not be used in any way with the research findings. We collect this information to be able to match your responses to your student's/son's/daughter's responses. Whether or not you participate will not affect your relationship with the school, your son/daughter's grades, any educational program, or your son/daughter's status at school. Teachers are encouraged to participate, but can choose whether or not to help with the survey without concern for their status in the school.
